Community Health Network Partners with Biofourmis to Advance Care-at-Home Services

Biofourmis, a prominent global technology-driven care delivery company, has been chosen by Community Health Network (Community), a six-hospital integrated health system in central Indiana, to elevate and expand its care-at-home offerings for patients across the care continuum.

In an effort to enhance patient care, Community will adopt Biofourmis’ technology-enabled care-at-home solution, replacing existing systems. This solution facilitates remote patient monitoring and care delivery, both virtually and in person, by Community clinicians. The focus is on closely monitoring patients and providing early interventions to mitigate hospital readmissions and emergency department visits.

Community plans to utilize Biofourmis’ solution to support smooth transitions for eligible patients from hospital to home, reducing time spent in medical facilities while maintaining high-quality care through telehealth, continuous remote monitoring, and AI-driven data analytics. Patient-reported outcomes via apps and bi-directional messaging will enable efficient scaling of care-at-home services across various diagnoses and clinical acuity levels.

Kim Jule, Vice President of Population Health Transformation at Community, emphasized the success of their Home Based Care services and the need for an experienced digital health partner to expand services across the region. Through a rigorous evaluation process, Biofourmis emerged as a comprehensive platform for care at home, positioning Community as a leader in facility- and home-based care within their market and state.

Biofourmis leverages licensed clinicians, clinical-grade wearable devices, FDA-cleared AI algorithms, and dynamic care pathways to offer enhanced clinical context for care teams. Community will collaborate with Biofourmis to seamlessly integrate the solution with Epic EMR and provide logistical support for patients throughout their care journey.
